All Issues Must Be Documented in an Issue Tracking System
In the ideal agile environment, when a tester discovers an issue, the issue is discussed with the developer face to face and resolved in as short a time frame as possible. The issue is only logged and tracked (perhaps with a yellow sticky note) if it cannot be resolved relatively immediately.
